This chapter of the Wildland Fire Education Handbook contains forms for reporting program activities to the School of Forest Resources and Conservation for a grant report in summer, 2000. You are encouraged, however, to continue to use the materials for many seasons beyond this report!

- Program Reporting Form Complete this two-page form to report your public programs, demonstration areas, and major media coverage generated by the Wildland Fire Education Toolkit. Fill out one form for each activity. (view PDF )

- Public Program Participants--Please collect the names and contact information of all participants as they enter your fire education public programs. This information will be kept confidential and may be used to assess change in participants' fire preparations. Final evaluation results will be shared with county teams. (view PDF )
- Public Program Evaluation--Please ask each participant to complete an evaluation form. If your program does not cover all elements, please direct participants to skip those questions. Multiple copies are included in the Toolkit. (view PDF )
- Additional Activities & Contacts--Complete this form to report on activities or client contacts where fire education occurs as a part of your other educational activities. (view PDF )
