This document is the Livestock Waste Testing Form (SL397) used by the UF/IFAS Extension Analytical Services Laboratories. The sheet provides details about collecting samples and testing fees. To download the form, visit

The Livestock Waste Testing Form must accompany livestock owners’ samples submitted to the UF/IFAS Extension Livestock Waste Testing Laboratory. This form allows for the following tests:

1. Standard Manure Test (pH, N, P, K, NH-4, % moisture, % solids, and % ash)

2. Micronutrients (Cu, Zn, Mn)
