Producer Soil Test Form

This document is the Producer Soil Test Form (SL135) used by the UF/IFAS Extension Soil Testing Laboratory. The sheet gives details about collecting samples and testing fees. To download the form, click the orange download button.

The Producer Soil Test Form must accompany soil samples submitted to the UF/IFAS Extension Soil Testing Laboratory by Florida agricultural producers when requesting any of the following tests:

  1. Standard Soil Fertility Test (includes pH, lime requirement, P, K, Ca, Mg, S, B, Cu, Mn, Zn, and also lime and fertilizer recommendations for selected crops.)
  2. pH and Lime Requirement
  3. Organic Matter
  4. Electrical Conductivity (Soluble Salts)

* Please note that crop code selections are limited to six per sample.

Florida homeowners and small scale producers should use the Landscape and Vegetable Garden Information Sheet (SL 136).
