Phosphorus Index Test Form

This document is the Phosphorus Index Test Form used by the UF/IFAS Extension Analytical Services Laboratories. The sheet provides details about collecting samples and testing fees. To download the form, visit https://edis.ifas.ufl.edu/pdffiles/SS/SS59600.pdf.

The Phosphorus Index Test Form must accompany soil samples submitted to the UF/IFAS Extension Soil Testing Laboratory. This form allows for the following tests:

  1. Standard Soil Fertility Test and Phosphorus Capacity Index Test (pH, lime requirement, K, Ca, and Mg)
  2. Phosphorus Capacity Index Test (Mehlich-3 extraction method) [pH, P, Al, Fe (capacity index of soil)]
